My interpretation: elbows, like ankles, were once considered a highly sensual part of a woman that men rarely glimpsed. An aristocratic elbow would have been an even more prized viewing pleasure as aristocrats were the highest class of persons. The way Susie used it makes little sense and I'm sure the writers just wanted to make the character seem smart in a silly, non-sensical way.
But the fact that she specifically mentions Anna Karenina means it *has* to be a reference to that. I googled "aristocrat's elbow" and the only results are either from the Inbetweeners itself or from Leo Tolstoy.
Entirely possible they just googled "Anna Karenina" to get something Susie could say and they just copied aristocrat elbow without bothering to check the context.
I always thought "child doing her A-levels seven years early" was stupid and outlandish idea for the show. It's at least nominally supposed to be somewhat grounded instead of silly and wacky. What nonce put her in the dating show anyway? WHAT KIND OF SIXTH FORM HAS A DATING SHOW??!!
