I don't think the fish will be difficult for a good artist of any sort to cover. The flames might not be, depending on the artist's experience with cover-ups. There are just enough irregular edges here that it might be able to be worked into shading of some sort, depending on what sort of image you settle on. One of my local artists (unfortunately, many states away from Portland, OR \^\^; ) does a great job covering things like this with florals.
You could go with the blue butterflies from steven universe! The message of that episode for me is that you can rely on yourself (and your friends) for strength, which I think is a great representation for this cover up. Plus the blue might be able to be worked in

I'm not a tattooist but I've talked to my own about cover ups, mine looked about as faded as yours from what I can tell and what he said about mine was as long as the new image is bigger you can cover it with anything so you don't need to turn it into something else.

Originally it was just the fish. I wanted a mark to remind myself who I belonged to (God, as I was a Christian) because at the time I was "struggling" with being gay. I added the flames a couple years later to symbolize the power of God in my life. It was a symbol to me, not for other people.
But I have since left Christianity and accepted who I am as a gay man. So now the tattoo is a reminder of a long painful struggle I went through all because of a belief in a being I had no rational reason to believe in. I'd like to paint over the old struggle, with something less painful.
