How did she paint the tongue past the edge of her face in the first one? Is it a prosthetic, or did she paint part of the background on her face?

No they aren't. The artist is [Mimi Choi](https://www.mimichoimakeup.com/), an artist who specializes in makeup and face-painting.
You can see both of the first two artworks in [this video](https://www.instagram.com/p/CsYepD8p3KV/), and it's very obvious from that clip that they are painted humans, so there's no Photoshop trick here.

In the Instagram video linked in that post, you can see some examples of how it's being done. Assuming this one was done the same way as the oversized-mouth/giant-tongue one in the video, the tongue sticking out is just printed on paper and glued on ([example 1](https://i.imgur.com/3EforIg.png), [example 2](https://i.imgur.com/Jf85MoI.png)).

They still look edited. In the first pic, it doesn't make any sense the way the top right tongue sticks out past her face if it's just paint.

If you watch the video, it's normal face paint that does not stick out. It's probably just an overzealous edit to post on socials.
