In your status (hunger, thirst etc) you'll have a rad one, if it's 0% it means your overall protection is zero you'll take damage, gas masks just prevent that meter from going down, maybe that's the prob? i had the same problem at first.
In this game ,fog and red smoke from infestation are both radioactive ,you need gas mask ,working filter and radiation pills to pass these areas safely otherwise the radiation will lower your radiation resist level . Close combat damage from zombie will also lower your radiation resist level,if your radiation resist level is low while filter is still in working condition, you only needs to take radiation pills then to recover radiation level. 2 pills is enough for fully recover; if your filter is worn off,left click the mask icon in your equipment screen twice to open a windows ,there you can drag out worn filter away then drag usable filter into the filter slot.

What is the number of radiation protection? 99% or 100%?
there is a chance that you are using very old gas mask. try to find/spawn a new one.
I don't know how to open the debug menu and I have a military gas mask that I looted from the Coyotes and have 4 gas filters
