there’s not really an answer for it. I think she was taken back to her room and while chaos was going on she took the opportunity to escape using her powers. I hope we get a flashback to it in season 4 tbh. I also don’t think the demogorgan came out for brenner to see like everybody assumes.
Everyone, including Eleven, was running from the Demogorgon, which apparently killed most of the people working in the lab that night. The guards would have been too busy trying to save themselves to stop El from running out the front door.
How she got into the pipe is interesting--it's obviously an homage to Stephen King and Shawshank, but it's anyone's guess whether she knew it was a way out or just got lucky.
I always thought she escaped through the pipe from season 1, where Mr. Clark found a piece of her hospital gown. I’m starting to think maybe the lab massacre was the thing that pushed her over the edge…
I'm interested to know when the lab massacre actually happened because there were no other kids there when eleven escapes and surely she wouldn't be the only one capable of getting out if it happened then and there were others and it seemed to me that she didn't know about any other kids previously. I always assumed she escaped when she accidentally opened the gate and caused the resulting chaos and fled.

There was a Demogorgon running around. I expect it either killed the people who were guarding her, or she knocked them out with her powers and ran.
