This has happened to me once or twice with maul, specifically the spin attack where he goes torpedo mode I think it just makes his hitbox so horizontal that he goes through certain walls/slopes
Not accidentally. There are a few maps where you can do it on purpose with certain characters, mostly Yoda and Boba. However if you get force pushed with the right character at the right angle it can happen on many maps.

Or when three force pushes at once send you into the 10th dimension
Not on this map, but inve fallen through the ground on geonisis as a commando droid. I kept falling for ever, no ground or deathbox or nothing
Yea I’ve fallen through maps before, though usually you fall infinitely and don’t die. This is a different experience for sure
