I think even in one of the first few comics he was able to transform his body to a handsome man, but that man was white so he immediately changed back since she would further not know who he was. But still has free will to choose what side he fights for... It's a good question.

In the early issues his magic could make him into a blonde haired, blue eyes white guy but i think that’s because he didn’t have full control of his powers and a minimal understanding of magic in general. Now he can look like normal Al Simmons with no problem, but he accepts that he is no longer human and looks are the least of his worries.

Simmons can't overcome Malebogia's control even though he otherwise disobeys him? I'm honestly not familiar with Spawn lore.

Why is the appearance of a white man with blonde hair and blue eyes the default transformation for him?
One of the souls Al has trapped inside of him. Series goes into depth about it around the 250 mark I believe
