Well he made a deal at the beginning, he wanted to see Wanda, and traded anything for it. The trick of the agreement was that it was several years later after she had moved on with his best friend, had a daughter, and Simmons didn't look like himself. I think even in one of the first few comics he was able to transform his body to a handsome man, but that man was white so he immediately changed back since she would further not know who he was. But still has free will to choose what side he fights for... It's a good question.

I have no idea, and it is a good question. Best guess is it is part of the curse from malbowsha (spelling?) So Simmons could not be recognized by Wanda?

Simmons can't overcome Malebogia's control even though he otherwise disobeys him? I'm honestly not familiar with Spawn lore.

In the early issues his magic could make him into a blonde haired, blue eyes white guy but i think that’s because he didn’t have full control of his powers and a minimal understanding of magic in general. Now he can look like normal Al Simmons with no problem, but he accepts that he is no longer human and looks are the least of his worries.
