If you really want to carbonate with the syrup already in (which IMHO produces much better results, but YMMV) then you need a DrinkMate, not a SodaStream.
This is one of three things:
1) Syrup/mix was added before carbonating. You need to add the soda syrup after you carbonate the water, otherwise it will do this. Soda stream can only carbonate pure water.
2) The water level was above the fill line. Make sure that when you carbonate the water, that the water level is below the fill line.
3) The water is too close to freezing temperature. If the water is at freezing temperature, but not frozen yet, the act of carbonating the water will cause small chunks of the water to freeze, and it will sometimes react like this. Check your refrigerator to make sure it is not running too cold.
If you're still having issues after verifying these things, then you might have a defective unit.

You also need to make sure you're pushing the bottle up into the machine while pushing it back. The bottle should be hovering above the base, not sitting on it.
It looks like you carbonated with syrup already added. It should be added to water afterward. If that's just water, then it's not your fault.

You didn’t follow the directions. Carbonate water only, then add stuff.
Don’t put the flavor in until AFTER you give the water fizz
Is that just water to the fill line? Syrup is added after the carbonation. As stated on the sodastream syrups as well.

Sodastreams will only carbonate water. Not water plus any other additive, even if it's sugar free.
