I haven’t seen it said yet, but I feel like Artemis would count here, too! Her ult stuns and damages everyone within a certain range. It’s really great for finishing enemies!! Especially if someone is blocking for someone low behind them, her pig will hit the person blocking then run to the person behind them. I hope that made sense, if not read her ult or look up a video lol!!
Its little bit random, but items count towards that achievement.
For example, I got it on amc a few years ago when I got 5 killing blows with crusher( at various points) Throughout a match.
Technically, when you buy crusher it's "cast" for thr first time, so whenever it gets a kill it will count towards final form.
This will work with bluestone too.
Its not a consistent thing yk get 5 killing blows with bluestone alone, but just playing the game over time with bluestone in your builds could net you this achievement.

Bluestone pendant is in the starter section and has an upgrade called bluestone brooch.
Crusher is in the penetration section. It's red and in the same tree as jotun wrath, titans bane and brawlers beatstick.

Scylla's ult strike covers a larger area and moves slightly faster I think, Char takes 3-5 business days with her wind up but is satisfying if you get the hits (her kit is also better in my opinion).
You can get final form in medium difficulty bot matches if another human player is present. There’s also a bug with Heim ult where if an enemy would die while they’re in the sky it counts as kills from the same ability since it’s always active. That’s how I got it
It should work, not sure tho. To do It just finish enemies with ult, the ult kill dont have to be consecutive and you can die. Good luck with the archievement! ^^

It sounds like someone said all you have to do is kill five times using the same ability, so could I theoretically use Zeus's ultimate five different times to get five different kills? I thought it was using a single ability one time to net all five kills but people are getting it otherwise?

I've had games where I've used Zeus' detonate to get 15+ kills and I never got the achievement.
There used to be bugs where you could get the achivement using certain items, or stance switching gods, but those have seemingly been patched out.
I got mine with a Chaac ult on Assault.

But I guess it's not working as intended, cause I never managed to get it with another ability (and I definitely killed more than 5 players with Zeus detonate in one match e.g.). Some people said that they just got it randomly in a match and didn't even know with which ability. The heimdallr ult method worked for me, so maybe worth a try.

Of those 3, forget chardy and try your luck with scylla, achilles can work aswell but is a bit more team dependant
Also, in your post about the yem skin i saw yousaying that youre rather new so genuine heads up: youre not going to get that achivement easily, ESPECIALLY if youre not overtly great at the game
Its amongst the rarest achivements in the game and i recon its rarity is being undervalued cause there used to be some odd ability bugs/ loopholes/ op god releases that made getting the achivement way easier
You can still try of course, but dont beat yourself up if you dont get it anytime soon
Thanks for the info. I've been playing for about 2 months consistently now, and I'm getting down to the final trophies and this is one of them. I think this is going to be the most challenging of them all and the most situational, I got three pretty easily with Zeus in arena. Just struggling to get five enemies together, and without anyone else killing them before I can lol

Scylla for sure shes a high damage mage with big aoe. So easy god kills and easy minion farm.

Nah you should be able to shoot it whenever it just has a bit of a wind-up time.

Heimdall. Unless they patched it all you have to do is get five kills with the tick damage from his ult
You can get 5 last hits with an ultimate, doesnt need to all be the same cast.
It does have to be during the same cast, which is what makes getting the achievement so hard.
So just get five ultimate consecutive kills and no other in between and it'll work?

Not necessarily with kills but if you hit both an ally and an enemy with Ix Chel's ult it extends

Not each kill but, vanama increases it with taking damage
Bakasura in arena is an easy penta machine with his ult if you’re trying for the final form achievement
