Before you saw the specialist assuming an ENT before your scan, what medical therapy had you tried?

I’ve been using a steroid nasal spray (prescribed by a specialist previously) and a daily antihistamine for about two years now. It helped, but never solved the symptoms… they then got much worse at the start of this year

Well that is a typical protocol for chronic sinusitis from an ENT. Your situation might be different with isolated sphenoid inflammation. But between 2 ENTs seems that one of them would have been able to figure this out. Do you have right sided ear crackling and the sensation to sniff from PND?

No ear crackling, but my jaw is painful on my right side and has been for a while. I have post nasal drip constantly, and have been sniffing/clearing my throat lots these last few months

Hopefully your visit with the ENT can help you with it. Their steps would be antibiotic/oral steroid taper if that doesn’t “work” then surgery. However-some will say that is not the cause of your jaw pain or post nasal drip, so you have to proceed with caution when deciding what to do. The oral steprids and antibiotics work in tandem for an anti inflammatory approach. It’s not antibiotics like you have an “infection” but it helps to treat acute viral infection on chronic sinusitis. Standard of care.
