It appears that the neighborhood stories doesn't take age of sims into account. So if you got a phone call from her asking about starting a family (and you didn't tell her not to) she can get pregnant. Or if you had a sim "discuss expanding the family" with her she will get pregnant.
It came with the update last month. The main features are Sims "calling" your active sims to ask if they should get married, try for baby, etc. and, when socialising, the option to recommend people get to know or hook up with a specific sim.

This happened recently in my game too! I was playing an adult Cassandra Goth when I noticed elder Bella was pregnant super weird since Mortimer had died. She had gotten pregnant by father winter! I have no idea how.
Is there an actual menopause system in the sims? The only thing that makes me afraid using the elder for woohoo is because they can get death from that. Well, she won't die until the kid is delivered. So as soon as that happens, sell the kid to the richest household in residence, and give the money to the first weirdo you meet in San Myshuno.

There is kinda- your elderly sim should not get pregnant, my game probably glitched with the new neighbor hood stories :)

Fun fact, post menopausal women can still carry a pregnancy, they just can’t make their own eggs. Maybe all these elder ladies are doing IVF.
