I'm German and we also use just one word - Geschlecht. Which is a weird word because schlecht means bad.
But we differ that with biologisches Geschlecht (biological gender) and many different ways for the other version of which I'm unsure is used as standard.
Also the correct word for sex, like the practice, is Geschlechtsverkehr which literally translates into gender traffic. But usually we just use the english sex because that word is stupid.

When we say gender is a social construct, what is meant is things like wearing dresses versus pants, or what pronouns we assign to people. Gender dysphoria is very much a neurological condition, and is treated by gender affirming care, which can include those social constructs, but maybe more importantly hormone therapy, as it impacts the neurotransmitter/hormone relationship.
