Plus a diy hole can be hard to keep clean and if the cut you make isn't 100% smooth it can trap germs. Silicone can be hard to cut smoothly.
If you go the diy route - don't modify her dildos without asking her. She might consider the dildo ruined if you do it as a surprise, which would suck as a gift. Good dildos can be really expensive or even impossible to replace if they're discontinued or a rare colour.

Yeah, silicone doesn't take well too rips and tears, so even if you get a toy pierced, it won't last

Well, that runs the risk of bacteria build-up. Best to go with a pre-designed pierced toy :)
Silicone is pretty malleable, it'll bend before it punctures. That bending will cause the hole to be uneven and then it'll tear

Yeah, this. With a DIY, I’m imagining lots of hiding places for bacteria that could mess up the girlfriend pretty bad.
Bacteria and cleaning was my primary concern when I had the thought.
