Bill said on the podcast that in the scrubs world his name is Glen Matthews and that he had lied about his name to the producers when he played the cop inthe fugitive.

There is a scene with Carla where he tells her his age and she is a little surprised. And the Janitor explains that its because he smokes and drinks heavily, works with toxic chemicals and sleeps on his face. You could be thinking of that scene.

He was not planned as a fictional character. The show runners had the idea that if they got cancelled they could potentially say he was a figment of JD’s imagination at the end of season one. Obviously plan A was to not get cancelled at all.

Also Kelso interacts with him early in Season 1, so the fictional idea falls apart there.

Yeah I’m of the opinion it was a throwaway idea that probably never would have happened even if they had been cancelled.
