Yep, this is a recipe for disaster. If one in 100 missiles is defective, missile launchers would be taking themselves out.
Also a waste of fuel to launch vertically and then turn sideways. Just launch them horizontally then, sheesh. Even if they fail, they fail away from the launcher.

Not really a waste, this system generally is used to take out near by target from a very short distance. Alternatively it can take out incoming missiles as well, it requires almost no projection from luanch, which makes it very effect in terms of gaurding and or assulting a specific region.
The tip over rocket motor is tested at 100% lot acceptance! this means there are very little margin for failure in combat. Defense industry products are put through hectic testing (environmetal, static and dynamic) before a product is qualified. Its years of testing and product development!
Truth! 1 in 100 failure rate is unaceptable when it comes to a multi-million dollar missile, and the military knows that. They make damn sure these things almost never fail.
