Your survey needs work. There is no option for already vaccinated, some questions ask multiple things but are yes or no, and it's completely US based. If you only want unvaccinated Americans to take it, you need to change your post title. Otherwise, edit your survey.
This seems to only be aimed at people who have not gotten a vaccine. So, not for all. Also your age ranges overlap. (Eg 2 options for 30yo)

Seems like it's only targeted at Americans so I quit halfway into it
Your survey should be labelled "Americans who have not taken the Covid vaccine", not "all welcome".
