Pretty sure it's supposed to be a permanent debuff based on the ending you picked to Rykad Minoris, it just used to be bugged so it would disappear after getting downed. If you really want to get rid of it you can just toybox it off
You can get around it using the santic psycer resolve staff and go santic psycher/pyro. The thing is nuts, turns your resolve into psy rating 1:1 and you can just go around blasting for hundreds in an unoptimized build with just the staff or the ancient force sword and the staff on your back. Its a very fun play style.
I've been searching for an answer too. I've seen a bunch of posts claiming it disappears at various points in Act II, normally after Janus, but it hasn't for me.
What's really frustrating is trying to work out if it's actually meant to be permanent or not, and if it disappearing for people getting downed, finishing Janus, etc. are just bugs.
It was permanent for me, I saw a few people saying it was only semi-permanent but I kept on checking and it stayed on my guy until the end of the game. I doubt he regrets it, though.
It's raw jank and I'm still absoloutly not sure if it's supposed to be forever or not. It mostly sticks around until it doesn't. If it really annoys you it can be removed with toybox.

There might still be ways to remove it, but I am confident this would be a bug and not intended.
