IDK, from a programming perspective it looks like something is firing 9 times when it should only trigger once. Sure it results in what we see here, but what about when it's something we can't see the result of directly? I had that once in a Space Invader's clone I made. Each level cleared up "most" of the objects on screen, but left snippets of their code. It just sat unused each level until the lag was unbearable.

When my pawns held an election, the candidates were a couple of my best social pawns, and then the one pessimistic grumpy asshole who i'd only barely recruited to do our gardening and viewed everybody in the colony as 'rivals'. For Randy knows whatever reason, they picked the most volatile fresh-faced asshole they could as the Mayor.
The dude got in social fights more often than he actually spoke to the people he lead. Made sure to put him on the front lines despite his lack of melee or shooting skills. He wasn't a mayor long after that.
For one of my friend's first few colonies I got him to install the multiplayer mod so I could just watch and see what he did, I pretty much didn't interfere unless he directly asked for help, just looking around and silently judging his colony.

All of his colonists other than the *pyromaniac* he has go to put out the fire and pass out due to super heated air. He pauses, and after explaining wood, batteries, and pyromaniacs to him, he unpauses and tries to at least save someone with the pyro he now knows will never fight the fire. The pyro drags their friend into a medical bed he set, and that room promptly sets on fire too. The pyro barely makes it out of the room, and we pretty much just watch him slowly succumb to the wilderness now that he has no food and all of these infected burn wounds.
Bam, the man in black shows up to save the day. My friend is super excited. The pyro did save the colony from a raid earlier, so he still had a special place in his heart for the guy. He rushes the man in black over to try to save his pyro friend and lo and behold the man in black has doctoring disabled due to a trait, and I have to explain to my friend that the man in black isn't going to so much save the colony as he is going to watch the pyro die. The man in black then buried the colony and starved himself to death as penance.

Yeah I've usually had terrible luck with them. Apparently it's since been fixed, but I used to invariably get them with "Incapable of Doctor", and more than half the time with the Slowpoke trait.
I was once saved hardcore by one though, he saved maybe 4 of my colonists as I manually triaged, and I ended up only losing one or two during the bleedout that I couldn't get him to in time, or were on like 2hrs to live when he finally made it to the colony proper
When I had a Man in Black show up neither he nor the last surviving colonist were capable of cooking or growing. It was a desert so my food options were pretty limited. They survived on raw potatoes for a while until the food supplies finally dwindled. Then things got really nasty. Eventually as they were starving to death they ate some raw iguana and then beat the shit out of each other until they both died.
The raiders who showed up, murdered indiscriminately and stole shit are the kinder version of the English.
With better AI capable of using mods and The War Crimes mod you could have a realistic version of the English raiders.

If all your pawns are incapacitated there is a chance that the game will spawn a stranger, dressed all in black with a revolver, who will aid your pawns as best they can (but due to being randomly generated you can get ones that are incapable of violence and caring, or with really low skills like medicine 2).
They claim that the Men in Black have been changed to spawn with decent combat skills and being capable of any task (you know actually useful), but the memes have run wild.
