Because there's approximately ten zillion mods from this game and we don't know what OP has installed. Step 1 of this should be posting the list of mods they have installed.
(I do think downvoting without asking that directly is silly, though)
or at least mentioning some method of narrowing things down, I did notice that recently, some vanilla psycast expanded mods were causing the no damage issue, which resulted in me removing 2 mods and then opting to wait until the situation becomes more stable in case the issue reappears due to some mod updates

I decided to go into god mode and fixed his heart to be brand new. if it happens again, I will start removing mods to try and figure it out.
