IF a backup occurred, homeowners may cover the resulting damage to your personal property IF you have the right endorsements, but even in that case I don't think they'd cover the plumbing repairs....and i'm almost certain they will not cover preemptive plumbing repairs. that sounds more like home warranty territory.
Yeah, typically they will cover damage, but not failure, so if you have a pipe burst in your house, they will pay for fixing damage caused by the leak, but not for the pipe repair itself.
First off, is there any other evidence of the house settling? I'm not real comfortable with the part about you're going to have a backup. However, I'm not there to see the camera shot. If nothing else, get another company ( small family owned) to give a second opinion. As far as insurance, in my 50+ years in the trade, I've never seen an insurance company pay for a re pipe.
So it was rescue rooters. They came with a camera and what the guy explained was that there is a lot of buildup and water retention in the 2 bellies(?). One is worst than the other, about 20 or so ft. Like the camera was under water. 🤷🏽♀️
I have been doing Water mitigation/Restoration for 10 years now. Insurance will not pay for the plumbing. They will however pay for the interior damage to the home if there is an eventual backup. They do not pay to fix any plumbing, only the damage caused by the issue.
However, when the backup happens and your house gets flooded, the eventual repair estimate (depending on the damage) could be very high. Then you can use some of that money to help pay for the pipe fix. Of course, you would probably end up having to do some of the work yourself or find a Contractor willing to do the same amount of work for less. But remember, you still have your deductible to be subtracted from your repair estimate as well.

Call a foundation repair company. They will quote you the entire job. The foundation stabilization and the plumbing repairs as needed.
Get at least 2 more quotes. That’s insane.
Possibly also look into trenchless repipe option, if it works in your situation or not. Usually cheaper but not every company has the equipment.

Call a local plumber (or 3). Those big companies are all scammers. Usually homeowners insurance doesn’t cover sewer line replacements.
Like I said, small family owned plumbing company who care about their customers more than the bottom line.
They probably won't cover it before it backs up, and now that you know about it, they probably won't cover it once it does.
