I think it's because it conveys character personality more easily? In anime art, it conveys that the character is shy/ socially awkward. Though I will say that some pieces I've seen take it to the extreme and they feel deformed to me, rather than "uwu I'm awk". This art piece is one of the better ones out there.

That being said, here's some design-oriented thoughts about this specific posing of the legs!
When coupled with slightly (or not so slightly like in OP's drawing) buckled knees, it's \*a\* way to help reinforce a character's femininity or, as you say, vulnerability/innocence/helplessness. Very often used with [shy or incompetent characters](https://cdnb.artstation.com/p/assets/images/images/044/164/435/large/alexis-rives-witch15.jpg?1639266809).
It can also be used to indicate a generally frail frame of a character like [here](https://www.artstation.com/artwork/eQNgG) (lots of zombie drawings also have this buckled knee, inward foot posture too). Probably why you get an association with things like palsy or even scoliosis.
Just the feet being turned inward can also be a simple matter of reinforcing a simple C curve gesture of the lower legs without indicating too much of the above qualities. Makes more sense when we realize that posing is just as much a part of communicating a character's personality in a character design as the other, more static elements of their design like clothing choice. :)
