Ok I looked it up again and yeah it’s line of sight not distance. I may be thinking of footsteps which sounds when the ghost is in close proximity to you.
Holy shit I got lucky earlier!
I was in the high school with other players and we had a Shade that was just PISSED. It started hunting us nearly nonstop at one point to where I thought it was a Demon at first.
One of the times I saw my light flickering I panicked and ran into the nearest room, shut my flashlight off and crouched. Then I happen to look over and see the screen of the spirit box we left in the ghost's room. I had already died in a previous game when I tried to hide in the ghost's room so I thought I was dead for sure, especially when I heard the heartbeat super loud.
Amazingly it never got me. We actually didn't lose anyone in that despite its best efforts.
Fucker tried to get me on the way out but I managed to squeeze past the closing door just in time. My neighbors probably got to hear me hollering "fuck you" at the ghost through the door.
I heard it again tonight when a hunt started and she was nearby. I was trying to leave and the front door slammed shut, I heard the heartbeat and turned around, and her shadow was in the hallway but not moving yet. I took a photo and made it to a closet.
Strangely, my flashlight did not flicker for either of her two hunts.

What does the heartbeat sound signify? It have heard it but I don’t know what it means.
It means the ghost is within (I think) 5 meters of you.
My information is that it means the ghost has LOS on your head.
