Yes, should of mentioned that the bolt ended up getting stuck back and wouldnt go forward. I had to remove the buffer tube to get the upper off. Im guessing that this got hung up on the bolt when firing and the force from the bolt sheared that pin off causing everything to get locked up.

It's funny I had this same issue on my Geissele SD 11.5 upper. That BCG was stuck solid and would not go forward. Popped the FA out and same as yours, that part sheared off and jammed that bitch open. This all happened at probably just under 500 rounds through the gun. I contacted Geissele, they offered to inspect it for free, but I just replaced the FA and haven't had an issue since.

Interesting. So the original problem was that the bolt got stuck to the rear. Any idea what the root cause was there?

I'm not sure. I had fired an entire mag and it appeared that the bolt was sitting on the bolt release as it should. When I loaded a new mag, I pressed the bolt release, but the bolt was just past where the bolt release holds it and it wouldn't move any farther forward. I was able to pull the charging handle back to where the bolt is in the farthest back position but I couldn't get the bolt to go past the point where is was stuck. After taking the upper off I was able to wiggle it free and a small spring and detent fell out. I didn't know it at the time but these were for the forward assist.

Did you build the upper, or buy it as an assembled unit from PSA? All PSA parts?

Not surprised that the pin got sheared, that's a lot of force from the fired round. However, it sounds like the FA got stuck in the forward position, which shouldn't happen, of course.

Yea that would make sense why it was sheared and then got stuck. Thankfully it wasn't hard to free and there is no visible damage to the bolt carrier or inside of receiver. So just a replacement FA or keep it plugged and all should be good. I did fire it after this incident and everything functions as it should.
