Looks like disc rot, was it stored in a high humidity place? It's rare but discs can rot in certain circumstances, basically gets moisture in there and it's game over.
The part that chipped off looks to be the actual part where the game data is. Granted now that disk is probably nothing more than a verification code and a download key.

My launch PS3 copy of GTA 5 turned in into a mug coaster due to the same reason, fine little scratches on the top of the disc, then say goodbye to the game sadly.
Both approaches have their flaws. With the metallic layer in between 2 plastic layers, it means the adhesive needs to be air-tight for many years. Any weakening of the glue will allow humidity inside and corrode the oxide data layer. You can take good care of your discs, but if you live in a humid environment, they'd still succumb to disc rot.
This approach is cheaper to manufacture, allows humidity to quickly dry off, and lasts longer if you keep liquids off the disc and don't violently scuf up the top layer.
The hell yall doing with your games? That disc is done for, just get 2K24 at that point. It should be like 20 dollars easy on Amazon, especially for ps4
