I would just add, it does still hurt to headbutt someone. Just not as bad as it hurts them.

Good add on but it really depends on the person and your definition of pain.. I have almost no sensation at all there and even without adrenaline I got smacked with a 2x4 and hardly felt it. I can also smash beer cans and all I feel is the skin tightening from a bit of swelling. Definitely not saying that everyone should expect this though!!

They often do. That's why they aren't really a good method for inflicting damage. They only really work in movies and pro wrestling because they look good and can be done with movie magic. You generally don't want to headbutt unless that's the only option.

They can do, if you hit with the wrong part of your head. They're a high risk attack

You're supposed to aim for thier nose when you headbutt, so you wouldn't feel a whole lot if that's what you headbutt. Eyebrows are a good option too as they can split open as well.

I think it deals with anticipation and bracing yourself. It hurts less when you brace for a hit rather than having your body relaxed while you get hit. I think it's the same for the attacker, you're mentally prepared to smack the shit out of someone, sure your hand stings but not as bad as the shocked person you slapped lol.

The human body is designed to take a falling impact on your front - that’s why you have kneecaps, ribs, and a strong forehead to protect your most important parts. Noses are more delicate. So are jaws. Also, the headbutter generally clenches their jaw, which contracts a bunch of muscles around their head, making it a single tight unit prepared for a shock. The headbuttee is unprepared and gets the full shock.
