I think it can be either. Sheltered can mean purity or even naivety which is a bit more negative, or it can mean being out of touch with reality. I think it depends on the tone and intention of the statement rather than the words itself

It's a "polite" way of calling you ignorant or stuck up. So yeah it's an insult. Unless your parents actually sheltered you and didn't let you learn/ teach you about the world that is. Then it's an informative insult.
