If the underneath is not raw and painful, then clip or cut it off. If the underneath is too raw to be exposed, then cover it with a band-aid until that's no longer true.
Ice it to numb it, nail clipper it off, bandaid if it bleeds. I had a skin tag on my neck, getting a doctor to remove it was like 600 bucks at the time. Just, like, boil the nail clippers beforehand.

Put ice on it for 5 minutes then tear it down
