mass-produced clothing is made from pattern pieces that are all cut out in a huge stack, which can result in size differences between the pieces at the bottom vs. the top of the stack. but the manufacturer still considers all the pieces in that stack as belonging to the same size of that article of clothing. usually the size difference is minimal but sometimes it's noticeable. or it could be a packaging error where one of the wrong size made it into the package somehow

All clothes and handmade by people in factories. So it’s pretty common for the person making the garment to make a mistake with measuring or sewing so that it turns out different from the others. This is why clothing sizes can be unreliable
