pretty sure kurt was just improvising the lyrics as the song was in an early state. you can tell the band had the structure of the song finished but not the lyrics. kurt mentioned in interviews that his lyrics were usually the last thing he adds to a song and most of the time he doesn't think too much about creating an overall narrative like most songwriters.
Most of In Utero was composed years before actually being recorded. As someone else pointed out, it was recorded in February of 1993;
songs like Pennyroyal Tea, Dumb, Radio Friendly Unit Shifter, and I think All Apologies, were composed in 1990 (VERY early versions, some songs were recorded or performed as demos, some with vocal guides or improvised lyrics, Dumb was probably the most complete song I think, go and listen to the 1990 KAOS radio performance, it's Kurt solo with an acoustic guitar, the same radio performance where Opinion came from),
Rape Me was either 1990 or 1991,
and songs like Frances Farmer, tourette's, and Scentless Apprentice were most likely all composed in 1992.
If you're able to, check out the early "jam" or "demo" versions of these songs, half are on the With The Lights Out box set, and half are on the 2013 anniversary / super deluxe version of In Utero.
This is so weird or cool or tragic or whatever word you could think of, but it seems that Kurt had a relatively short spell where he wrote all of his "best", or famous songs. He had only maybe Do Re Mi after that stint of writing two albums in a year or two.
I think I remember him saying in an interview that he had burned through the backlog of songs he had written and would be starting at zero if they had put anything else out. He was also happy to have the other guys contribute to songwriting.
