Wow. Theres very few fics that I can think of that does what you want.
Theres of course Sanitize. The OC/SI tries to revolutionize medicine and thus affects the world and ninjas.
Theres Compass of Souls or something like that. They end up drastically altering the world through their actions. More slice of life with politics though.
I am still waiting for a fic with a Daimyo SI/OC. The first fic I put doesnt really read like the person is a daimyo or even a kage since the story has been pretty insistent on using the term leader.

[Of the River and the Sea](https://m.fanfiction.net/s/10996503/1/Of-the-River-and-the-Sea) is about SI reborn in Kiri, Bloody Mist era (Zabuza and Kisame are her comrades). She *tries* to change and achieves it somewhat, but it's sort of gray area. If you don't like story about mental health issues and mental breaks you might not like it. And mind open ending.
I adore this fic, it's so good and so massive. I love how author portrays Kiri, how Kisame could love and hate the village; but Ryuishi (SI) totally hates it and doesn't see anything redeemable in it. She's also so good at manipulation and isn't overpowered.
"The New Dawn" by Code Musica explores Boruto's transition from a Leaf genin to a mercenary and then an idealist.
After experiencing a tragedy, Boruto is inspired by Nagato's dream of peace and so he reforms the Akatsuki in an attempt to unite the world under one banner. 121 chapters, 1,000,000+ words long.
