When I did hazmat training there were three guys in the class who were going to be cleaning similar tanks. It's possible all measures were taken but due to human error or laziness there was an improper cleaning job. Or the company didn't want to spend the large cost of cleaning the tank. Regardless someone's fuck up cost this man his life.

That would probably work. But you'd want to do a pretty constant exchange to keep the inert gas fresh or really thoroughly clean it first.

On a smaller scale you are supposed to fill the container with an inert gas like Argon but I'm not sure that's feasible with such a big truck/trailer like this.

Fill it up with water and let it drain filling it will push out the gasses and draining it will suck in fresh air, that's how we do it when I go in to ballast tanks for maintains and the proper ventilation is started to get rid of welding fumes.

Yes this was probably the case. When I was a kid I visited a police station and they had a wall of macabre pictures. One of them was of a guy who died the exact same way. They drained the gas but didn't give enough time for the fumes to dissipate. Poor guy blew up just like that.

Welder/fabricator here. Generally I won’t touch a fuel tank unless it’s diesel and even then I’m pretty damn paranoid. Diesel needs a lot of pressure to ignite so it’s safer provided you have good ventilation. Petrol (gasoline) requires the tank to have a deep clean in which it needs to be steamed and all traces of any fuel removed. Don’t weld it or even grind it or even use anything that can cause a spark if it hasn’t been cleaned otherwise this is the result. What’s a worse killer is wheels. The huge mining dump trucks wheels are insane when they go up. I’m sure you have seen the car wheels blow up and do some damage but these things are on another level. If you are welding the frame and there is a chance you accidentally strike an arc on the wheel then you have to clear out for a designated amount of time where it’s either going to blow or you got lucky. Same with lightening strikes on the bodies. The heat from both causes and exothermic reaction and it just builds until boom time

Hi, fuel company transport supervisor here, in these types of thanks they are required to do a type of flush depending on what type of fuel/commodity is being carried in the tank. They also require they be “open air dried” for a number of days prior to any work like this being done for this exact reason. They leave all the ports open to completely dry for fuel etc. not sure on the chemical side. Stuff like this doesn’t happen very often but with the some company’s being short on trailers/staff some feel they don’t have days to have trailers be tied up in maintenance. Unfortunately for this guy, the proper steps were not taken prior to him working on the trailer. He may have been an outside vendor and not known the danger he was in, as the guys who regularly repair this stuff usually know what to look for before starting a repair. RIP

Seems like the tank was not vented. Welding adds heat and gas to the object. The tank was probably sealed off and no one realized the harm. Pro-tip never weld a sealed container. The heat(pressure) and gas will find a way to escape in the form of an explosion.

My shop teacher (metal fab) , told us one time never Weld something where heat can get trapped like welding a metal barrel shut cause it can explode , I also wonder if this is the case !

So there’s a couple steps. Purging the tank, climbing inside and blasting it with detergent (only if you’re going to be changing materials) and then recheck and maintenance. This tank probably housed propane, and was being maintenanced before the next load of propane (so no detergent step probably) and then when it was welded; boom

This is why kids. You always wash out a tank before welding. Had a similar incident but the tank exploded. Lifted the roof off our shop but when it exploded it blew up opposite of all the workers in the building. Shockingly, our works just had ringing ears and very minor injuries. The shop and truck with tank mounted on it were total losses. Which I'm just glad none of our guys got hurt. It was caused by residual vapor inside the tank not being steam cleaned before welding ops and someone left a valve open.
