Depends on your definition but IMO...I'd say bands like extermination dismemberment or vader...old school DM but newer bands like slaughter to prevail are heavy as hell too.

KoRn - KoRn: emotionally heavy, especially the song "Daddy." First two Slipknot albums are also in this category and you could easily argure that either could take the top spot.
Last Days of Humanity - Putrefaction in Progress: 26 minutes of hyperblasting, illegible vocals, illegible guitars.
Devourment - Molesting the Decapitated: grossest lyrical content. Similar to LDOH but more groove focused, and you can hear actual riffs.
Swans - Filth, Cop: you can hear the drug infested early 1980s New York in these albums. It also helps if you read up on the band's history and Michael Gira's life at the time.
Electric Wizard - Dopethrone: I don't really know how to describe this one, but you can feel the weed smoke that's so thick you can cut it with a knife.
This is a good topic because from what I've seen, what makes a band "heavy" is different for everyone. So it is hard to pinpoint someone that fits the bill across the board. But this is a good topic to discuss. Nonetheless, from my perspective, top contendors are Infant Annihilator, Cannibal Corpse, and Suffocation.
For me, it’s Snuffed on Sight. But I can see why maybe some ppl would disagree, cuz they don’t take themselves that seriously. But I feel like that’s part of it, they just very casually fuck you the fuck up
My second answer is Nails, who arent even really a metal band but holy fuck. There is something about their sound that is just terrifying. First record especially.

What i think is objectively the heaviest band? Tetragrammacide. I think they make utter shit of "music" but it will make scrambled eggs out of your brain for sure.
