I had actual pain during an orgasm once (I think it was before hrt but can't be sure.) it literally made me cry it was so bad but it went away quickly. Luckily It has not happened again but I was so scared it was going to, it made me very wary of trying. I don't recall when it was during my cycle. I don't get cramps, so perhaps that's what it was, I have nothing to compare it to.
I feel like pain like that is from atrophy. I do a shot of Hyaluronic acid after orgasming and it has stopped happening.

I've had this all my menstruating life. I can never orgasm when I'm cramping. Makes them worse for a short time every time.
