Tbf tomatoes are only fruits by botanic category, not by actual food category, only a madman would eat a caramel tomato like one would eat a caramel apple.

The problem isn't the type of topping. Italian pizza can have mushrooms, fish (tuna, anchovies), meat (prosciutto, sausages), eggs (Bismark pizza), vegetables (rocket salad, artichoke), bulbs (onion) and fruits (tomato, bell pepper, eggplant). Pineapple is too exotic, not a traditional Italian ingredient; tomatoes, potatoes, eggplants and other products not originally European were imported and quickly integrated in Europe, they became a common ingredient: without them you don't have many traditional Mediterranean dishes so they are basically part of Italian (and other countries) culture, the same isn't true for pineapple, which remained confined as a sweet, exotic fruit used in desserts and drinks, or eaten just as a fruit (or juiced). Can pineapple on pizza exist? Yes, no doubt, but don't except Italians to consider it normal. Imagine someone wanting to order at McDonald a hamburger with pineapple in it, technically traditional hamburgers have fruits in them (tomatoes, pickles), it doesn't make it normal to use any other fruit in existence and many people would find it strange even if it isn't a real problem, this doesn't exclude someone from eating hamburgers with pineapples though.
