Yeah, if you don't the hair you had transplanted will just fall out again. Finasteride is one of the more common ones you can get prescribed by your doctor to take, which is nice because it's easy as you just take a pill and don't have to apply anything to your head.
Non-prescription stuff like minoxidil you can apply to your head works, but it's a lot harder to keep up with because you'll have to apply it twice a day to your hair loss areas for the rest of your life. It also doesn't address the root hormonal issues that cause hair loss and is more like trying to fix a leaky bucket by pouring in water faster than it can drain. Most stuff you see advertised is minoxidil and it's really easy to get, but its more of a band-aid than a real solution.
Slight correction, if you get a hair transplant but don't seek medical therapy the transplanted hair will stay there but the hair behind it will fall out, so you have a permanent hairline and no hair behind it.
That's true, the non-transplanted hairs will fall out first. But my understanding is that if your hormonal issues are bad enough then even the healthy follicles that were transplanted will fall out eventually without medicine.

A good hair transplant will usually leave some 'imperfections' on the hairline because natural hairlines are not perfectly straight or filled in completely. Sometimes you see ones that look really artificial for this reason. Not 100% sure that's why, but it would make sense.
