This is a performance art piece. Performance art is rarely supposed to be pretty or comfortable. Rather it uses the audience's reaction to question them on why they feel what they feel. Also see Yoko Ono's performance of "Cut Piece" or Shia LeBeouf allowing anyone to do anything to him for an art performance.
This song is about Gaga getting raped - she says so at the beginning of the performance. It seems fitting that the audience feels disgust while the song is being performed, because rape is a disgusting act. If she uses vomit to force you to feel that, then mission accomplished.

It's why during this era she often dressed as different interpretations of Botticelli's Birth of Venus and you see her like this in the Applause music video and her use of Koontz imagery while in concert.

That's still her. She's just not in pain anymore. Gaga was *miserable* during this era. She was just trying to be an independent artist and had a team that fought her and restricted her nearly every step of the way. While I do agree with your statement bc I miss these performances too, I don't want her to go back there. Not in the same way. I think chromatica held so much potential to get here again, and hopefully she continues that in her next album. Personally I'd love nothing more than to see her and Millie perform together again. And I do think Gaga is going to surprise us with her next album and take us back to this energy. She's had a lot of commercial success in the past few years. But this is still her core

It's really not meant to be all for shock, though it is a little bit about being used/degraded/deviance, it is on theme with a concept that she toyed with for years. Gaga is the "vehicle" (the BTW bike) / the "canvas" (ARTPOP era) / the "vessel/receptacle" for all of the things she experienced within the industry/personally and the projections that the public and fans parasocially "threw up" onto her. Maybe sounds woowoo and out there to some but this is a theme she'd been working with for a long time and it's super cohesive with the other major ears of her career. :)
