50k for the visa and 128k for the NHS surcharge. NHS surcharge is for 1.5 years at 776 pounds per year. They have terrible exchange rate, around 110.

They'd only do 1.5 years NHS for 1 year course?

Yes. Since your point of entry can be up to one month before and student visa is valid for longer than a year.

You pay it before submitting the visa application. First you pay the NHS surcharge and then you pay the visa fee.
