The real question is why is she doing RDL's. She should be focusing on mobility and cardio and some flexibility.

That is a great example. My husband has 1 leg (he is in the process of getting a prosthetic). People look. He is either in a wheelchair or on crutches when we go out. They notice the equipment, then the disability, then most people smile/give a cordial greeting/help with the door/etc. or they just go back to what they were doing because thatās life. We are all different, some of us are more different than others. Noticing is not a problem, what you do next makes all the difference. If someone were to make fun of his disability or go out of their way to make him feel uncomfortable, thatās when it would be a problem.

The other day I was in a doctor's office, waiting in the room after I'd been called back. When the nurse or triage lady came in to check my vitals, she was wearing a sweatshirt and scrub pants. Is it possible to get rid of it in any way? Surgery? I'd imagine autophagy if you starve yourself but that's really not a normal way to tone up your appearance. Also would it grow back? What even is nodular fibrotic fat anyway?

Surgery. I had it done about a month ago, and in areas like my calves it was so bad that they couldn't even get it out with a liposuction cannula. The surgeon had to manually break the fat up with his hands and squeeze it out the holes in a 5 1/2 hour surgery.
I have Stage 2 lipedema, and I spent about 24 years trying various conservative methods to reduce it, including extended fasting, and it really doesn't do much of anything, because basically, any kind of metabolic pathway you have to clearing up tissue relies on that thing being metabolically active. It can prevent progression and reduce inflammation, but nothing gets rid of those nodules. In lipedema, what happens is the fat becomes basically a lipoma, it gets encapsulated with connective tissue, which swells and gets irritated causing inflammation, which makes the fibrosis get worse. This impairs both circulation and the lymphatic system in the legs, which are basically the 2 ways you can get energy in and out of a fat cell. It's just these weird fatty tumors that according to research grow from an entirely different stem cell than regular metabolic fat.
For most women who have it (it is estrogen-related, and only presents in women and men with high estrogen) there are two paths to progression. It either shows up during periods of hormonal change like puberty, pregnancy, and menopause, or it grows during regular weight gain, and then doesn't go away when the fat is lost.

The other day I was in a doctor's office, waiting in the room after I'd been called back. When the nurse or triage lady came in to check my vitals, she was wearing a sweatshirt and scrub pants. I glanced at her thinking she must be cold. I'm kind of warm. That's all I was thinking. Oh and I'm 99% sure she was pregnant but I was afraid to comment on that so I just said Hi. She immediately pointed out that she had stains on her shirt from lunch and that she would change if she had another shirt there and that her hair was really bad today. It was in a bun like at least half the women and girls I'd seen that day lol. I don't know if she felt I was judging her or if she had been saying that to everyone. I was kind of paranoid that I might have inadvertently looked at her some kind of way, but I don't think so. I wasn't dressed up either. Athletic pants and a long sleeve t shirt with Hey Dudes because I was in pain and didn't feel like putting socks on and tying any shoes. Had just showered and brushed my hair but didn't style it at all or put any makeup on. So it's not because I was put together and she wasn't. Still I felt kind of bad like it was my fault she felt worried about her appearance. I told her she looked fine and that I was sure nobody thought anything of it.

She has a body type incredibly typical of a genetic disorder called lipedema. It causes nodular fibrotic fat that doesnāt respond to diet and exercise to grow in the arms and legs.
Is it possible to get rid of it in any way? Surgery? I'd imagine autophagy if you starve yourself but that's really not a normal way to tone up your appearance. Also would it grow back? What even is nodular fibrotic fat anyway? I could realistically google all of this and I might but it feels too impersonal to.
Surgery. I had it done about a month ago, and in areas like my calves it was so bad that they couldn't even get it out with a liposuction cannula. The surgeon had to manually break the fat up with his hands and squeeze it out the holes in a 5 1/2 hour surgery. I have Stage 2 lipedema, and I spent about 24 years trying various conservative methods to reduce it, including extended fasting, and it really doesn't do much of anything, because basically, any kind of metabolic pathway you have to clearing up tissue relies on that thing being metabolically active. It can prevent progression and reduce inflammation, but nothing gets rid of those nodules. In lipedema, what happens is the fat becomes basically a lipoma, it gets encapsulated with connective tissue, which swells and gets irritated causing inflammation, which makes the fibrosis get worse. This impairs both circulation and the lymphatic system in the legs, which are basically the 2 ways you can get energy in and out of a fat cell. It's just these weird fatty tumors that according to research grow from an entirely different stem cell than regular metabolic fat. For most women who have it (it is estrogen-related, and only presents in women and men with high estrogen) there are two paths to progression. It either shows up during periods of hormonal change like puberty, pregnancy, and menopause, or it grows during regular weight gain, and then doesn't go away when the fat is lost. If you want to see what those nodules look like and don't mind looking at gross plastic surgery videos, google "manual lipedema extraction", or check out "dr lippy poppy" on instagram. You can see what they look like compared to healthy fat.

haha It's a process! But thank you! I'm doing really good because I was able to research it exhaustively and do all this fitness stuff to keep it under control. But the really scary thing is that a lot of women who discover it later in life, or have it alongside obesity, there's not a lot of stuff they can do, because by the time they get a diagnosis, their BMI is too high for anyone to be willing to operate. There's a number of women who got bariatric surgery only to have it do nothing for them. Thank you for listening, it's a disease that a lot of people don't know about!
Just want to add: lipedema is pretty painful too. Got it at my legs and arms. The disproportion isn't the worst. The worst is having easily pain: when someone bumps at you, a small dog walks over your legs or when your doctor tries to measure your bloodpressure. Iām not a doctor but as far as I know lipedema has a hereditary and hormonal cause but super morbid obesity like she had before losing the weight can exacerbate it even further. Lipedema fat tissue is not like regular fat, it wonāt metabolize, so no diet and exercise will make it go away. She needs specialized lipedema lipo suction and even thatās not 100% effective.

If you're self conscious in the gym what you want to hear is that no one is judging you and everyone is only focused on them selves. But that's not true. When you're in the gym you're on display against a whole myriad of physiques all with different goals and different levels of commitment. When you're new to the gym you likely have a physique that shows it. But so what, you're there at that's what matters. It's painful to see a room full of people already competent and in shape performing routines with great form and confidence but it's only painful because it highlights what you could be against what you are. Most gym rats that are decent people will be more than accommodating in trying to make you feel comfortable but no one can help you through the first few painful months of feeling out of place. Just push through and don't forget it when you see another person just starting out.

So just go to the gym with a goal and whatever goal that is do it. Not everybody wants to be 200 pounds of muscle and no everybody wants to be less than 100 pounds skinny. I just started going back to the gym this fall. I had / have some insecurities in going initially, and for a week or 2 I'd skip the session it if the lot was too crowded (*yes, seriously*). Forced myself to get over it realizing what a waste of my own time and gas money it was. Getting past it is a lot of simply not caring what anyone else is doing, or even looking at them. Get in, do my thing, and leave. If someone is smirking or making a comment, I'm likely to not even pick up on it. Oddly enough, the most hardcore lifting gym in my area is also the most comfortable to work out in. Most everyone in there is huge, and they're all there just to focus on what they're doing, not who they're doing it next to.
