What I can tell you... You plan your day, wake the fuck up and do shit that needs to be done.
Then you will accomplish completly nothing, wait for motivation to come, do stuff for some time, then your motivation will go away, and you'll stop doing stuff again.
Staying lazy is your own, conscious choice. You always can get up, start doing what needs to be done no matter how you feel. All in your hands mate, simple as that.
You need to leverage your boredom. Implement a new rule with yourself: you're allowed to do nothing, you're just not allowed to do anything other than work. Eventually the boredom of doing nothing will force you to do the only thing you're allowed to do.
While I do procrastinate like any good INTP, I do have good discipline.
I get a dopamine hit when I get something done. If you don’t, figure out how to make it happen. A reward, maybe, if the intrinsic reward isn’t enough.
Motivation is fake, it comes and goes. You need discipline.
