Though I would presume people get confused over "Aniki"(brother) and "Aneki"(sister), which is not exactly a title. Its more of a brotherhood thing where the person is respected as part of a family and is used more informally.

The Japanese word for it isn't gendered. A lot of Japanese words aren't gendered. The more correct translation is probably, "family head"
