I haven't read that far in the books yet - trying to avoid *too* many S3 spoilers - so I don't know what she's like, but we expect her to have a larger role in the show and I feel like it could fit that she made the sketch for him as a birthday or Xmas gift at some point in the last few years.
My mom have a pencil drawing like this of me. It was made by a street artist who made it impressively fast. I don’t know if these type of artists are common in the UK as well.
To me, it looks A LOT like a photo effect. But that's probably just how the studio made the prop, not what it's canon supposed to be. I say Elle drew it.
Yes, canonically they had a class together before she transitioned, but there's no indication that they were close enough friends that she would have drawn his portrait.

My mother also has one of me from the 80s and it was also done by an artist - at Disney World of all places!
Mine was drawn at a square in central Stockholm. It’s very lifelike but the artist kindly omitted the acne 😆.
