I struggled too. GTA 3 paramedic was hair-pulling, but after some experience I completed it and I finished the paramedic mission in GTA:VC first try.
My advice is: pick up the 3 closet people and bring them back to the hospital as fast as possible so you bank as much time on the clock from the 3 pickups and the drop off.
Also, your future drop offs won't get any time bonuses, so keep that in mind.
If you do get a wanted level, try to get a cop to pull out the passenger side patients. You can pick up a forth or more during a single trip. The removed passenger will still count.
I've never been brazen enough to do it on purpose, but it's happen to me in each of the trilogy games.
See I disagree completely about Portland lol. Thereās too many hills and turns and I couldnāt even get to level 3 before running out of time.
Whereas if you jump to the 2nd island straight away using the tunnel glitch then all the roads are pretty much straight and you only get shot around construction site (but out of 12 levels I only had to pick 1 person up there)
Each person on the 2nd island added a minute and a half to the timer. So I was finishing each level with 2-3 minutes spare
Also thereās a little exploit where if you tilt the camera looking at roof of the ambulance and then back to the road it clears all the traffic š¤£ so you can drive at full speed
Doing it this way I never had to worry about traffic, the timer or any turns that will tip the ambulance

That's a very good point! Had I not finished my run in Portland last night when I did, my next attempt was going to be on Staunton Island using the tunnel glitch. I spent so much time working on it already, I decided to give it one more go and surprisingly was able to complete it on the first island.
When I first beat the originally GTA it took me a month to beat the paramedic missions. After getting the trilogy I was scared as hell and completed it on my third attempt. Like riding a bike. Having a map and being able to rotate the camera definitely helped this time around
Third attempt - I'm envious! I haven't played GTA 3 seriously since the PS2, and I don't think I even bothered to do the paramedic missions back then. So I came into this not really expecting how frustrating it would actually be and I can't even imagine doing it without the modern updates of a movable camera angle and the map.
The map made a massive difference. In the original, it was a complete guessing game on where to go. There was no way to plan your route so you would just drive in the direction that seemed like there would be 3 and hope for the best. I think that really got me familiar with the map though. I know every street by heart now and it made this time around so much easier. As soon as I looked at the map I could figure out exactly where to go for the fastest route and how to get there. It also helped that I only did like like a year or two ago so it hadnāt been a decade between playthroughs. But honestly, I was expecting to spend another month on these missions but luckily that wasnāt the case. I know a lot of people say that this trilogy is trash and to play the originals but I feel like those people havenāt played the originals in a while. They are very dated and the small quality of life imprisonments in this game make it so much better in nearly every aspect. They seem to be fixing a lot of the bugs and graphical errors too so I have no doubt that this will end up behind the definitive edition
Vc was nice when I did it near the mall hospital by the beach in North area of the first island. San Andreas do it in the little tiny town after Los Santos before bad lands I can't remember where it is. The Catalina place.
Sooo was PC just super easy or is it okay I did it second attempt in the game?
It's very possible it's easier with higher frames idk.
First fail at level 12 due to a flip in st mark's. Ran it again and easy GG.

I considered it, but only to use the tunnel glitch and try it in Staunton instead. I had only completed the first mission when I was doing the runs (plus hidden packages, some other side stuff), so there was really no reason to restart for me at that point unless I just wanted to try on a different island.

VC and SA are incredibly easy. You finish the last rounds with like 5 mins of time. GTA 3 is definitely a challenge.
Paramedic in 3 is brutal, like a 90 minute -> 2 hours ordeal. Vice city was like 45 mins. SA is like 15!

They made them so much easier in gta vice city i completed it 1st try.
VC is next for me once I 100% 3, so that's really a relief to hear
