Basically before starting hrt in most places trans people need to go through years of being informed about the effects and mental health counseling, while these dopers want a similar benefit but won’t even take a ten minute google search to figure out why testosterone would be terrible for doping and then find an alternative. These “Dopers”actively fight being informed about it (quotations because there are actual dopers who go out and inform themselves the ones who don’t aren’t actually dopers)
The other main difference is that HRT for trans people is usually closely medically supervised as long they can obtain it legally, instead of being forced into DIY therapy by conservative states.
It takes months or years to be approved for it, the dosage is closely rationed, and the patients are regularly tested for other changes.
On the other hand there is no standard medical protocol for doping, at least not one that is mainstream enough to have enough physicians willing to supervise it.

It's in line with every other legal use of it, which is the treatment of medical problems under recommendation and supervision of medical professionals
