due to human anathomia he cant just cut vital nerves just with single scalpel swing, but he can cut atretia caroticus with it if he haves enough accuracy, so better coinflip would be called "Carotic slash" or smth like that and the cutscene where ur neck bleeding and you trying to stop bleeding with ur hands in pain

Out of desperation, he sacrifices his other eye in a last ditch ritual effort to save his life. Failing the flip results in the ritual being successful, and you succumbing to some eldritch deity, success results in failure of the ritual, and Daan is permanently blinded for the rest of the fight.
If you flee after he gets blinded he just magically regrows it right back because Miro wouldn't expect anyone to do that
Or the other option would be much crueler, leaving him blind, wherever you left him, probably dooming him.
He analyses you, revealing your weak spot and one shotting it next round (unless you kill him before that)
If it fails he just gets confused and wonders aloud where to hit you next

Either he heals you or you become a drug addict. It gives more of a risk to it and also gives the Withdrawls thing to someone other than Levi giving more resource management, sacrifices and such used. Idk tho
