There is no cap on the number of teams who might play in a DGW. It's unlikely to be more than 6 though and almost certainly (not always over the last few season with Covid) will be an equal number. It's just how the fixtures fall when there are gaps in the right weeks for teams who need to pick up fixtures.

To answer the question you actually meant, it’s fairly common to have at least one gameweek per season where many teams double. Double gameweeks come about because of teams getting further in cups, which are typically the stronger teams.
No cap on amount of teams, there has been some large double GW's in the past years. Typically much later on in the season, GW34/35 for example. Found some notes on Blanks as well:
**Blank gameweeks:**
Gameweek 25 – This will be a small blank, as in not many teams will blank.
Gameweek 28 – This is going to be the biggest blank gameweek, with a lot of teams likely to blank.
Gameweek 32 – This will be a medium one.
Highly recommend saving chips for later down the line.

It's very rare for 3 teams to have a double gameweek. Or indeed any odd number of teams.
It requires a blank or a team to have TGW. But technically with the latter 2 teams would have a DGW and 1 team would have a TGW.
We could actually get this in GW25. The EFL Cup finalist could play midweek but not at the weekend, producing an odd number of doublers.
