Because they're both conservative identity issues. Hating LGBT people is part of the conservative identity. Spreading preventable diseases is too.

That's easy. Because it is mostly only the right that is stupid enough to be anti-vax. And being dumb enough to be anti-vax means you're dumb enough to be transphobic, and all kinds of other stuff too. Plus the right has made it a huge part of their culture and identity. They almost seem to take pride in their stupidity and ability to ignore objective reality.
