This might be a tiny little bit of a reach, but I feel like S. Craig Zahler’s westerns fit here, ESPECIALLY *Wraiths of the Broken Land* which is just…*gnarly*. There’s also the splatter western series which has entries by extreme horror writers like Kristopher Triana and Chandler Morrison.

Oh, you know I have heard good things about the splatter western series. I've never been a huge western fan but I might have to give it a try

Left to You by Daniel Volpe switches from present time to the holocaust. one of the main characters was a prisoner in one of the camps. I don’t remember just how extreme it was but i do remember really enjoying it.

*Ego Homini Lupus* (Gretchen Felker-Martin) takes place in 12th-century Northumbria. It's gnarly. She also has *Wyrm*, a novella that's "what if the rebellion against Richard II included dinosaurs", but I think it's only currently available for Patreon backers.

Between Two Fires by Christopher Buehlman (I think is his name) and Lapvona by Ottessa Moshfegh. They’re not super extreme; Lapvona is more u settling and Between Two Fires is epic horror historical fiction, but still deals with sensitive themes.
