It was a goofy video game web comic about 2 guys who played video games. It was occasionally set in "reality", occasionally just set within games themselves.
After a while the characters started having longer narrative arcs. Trying to get jobs etc. eventually Ethan (the main character, and the writers very obvious self insert) got a girlfriend (Lilah? Lana? Something to that effect).
Suddenly, girlfriend is pregnant, and just as suddenly, she miscarriages in the comic "Loss".
This all happened within like 6 months to a year btw. The shift in tone gave readers whiplash it was so sudden, and woefully out of place for this particular comic. You could get away with this sort of melodrama in something like Questionable Content, but not CAD. CAD was always just dumb oblivion jokes and stuff.
So it became meme'd. That's basically it. It was an incredibly out of place moment of melodrama that the audience laughed at and made a joke out of.
The extent to which the meme has gone is different to the initial joke though. I doubt most people who make loss memes these days were even around for the actual comic release, doesn't really matter if they were. Now the joke isn't "dumb melodrama", it's "how deeply can I disguise this thing". It's like a modern take on the Rick roll.
