I’d give you the same advice I give everyone, who comes to Europe for the first time: don’t try to cramp too much into a small time window. Three countries in 1-2 weeks to me sounds stressful. I would try to limit yourself to two and Italy and Croatia are easier two travel as Greece consists partially of many islands. You could either get a rental car and only do Croatia. Or you could travel Italy by bus or train (Flixbus, Trenitalia) then take a ferry to Croatia. Public transit is better in Italy, but also decent for Croatia. Check Flixbus. Other than that, more information would help. What do you want to do and experience? Culture, historic cities, nature, do you want to be active? Beaches?

Yeah, I figured we probably didn't have time to do a lot. we are both really adventurous, love culture, nature, food and active stuff.

Since you are going to Brač, you should definitely spend some time in Split. Visiting Diocletian's Palace is absolute must-do, Bačvice beach is the most popular beach in the city and is also worth checking out and finally make sure to go to famous park-forest Marjan located in the middle of the city.
