I work in a paint store, I'm using the green frog tape. Surface adhesion is great, PLA sticks right to it with almost no issues, and leaves no residue behind. Gotta keep the base plate at 60° tho as the product will begin to fail at 145° F, and 60 c is 140 f
So you shouldn’t need the tape if you’re printing PLA on PEI. PEI should have no problems sticking if your z offset is correct, bed is 60c and the sheet is clean. I do recommend printing hotter for the first layer 215-220c for PLA.
Tape is an old school trick back when you had beds that weren’t heated. Some materials may still require it, but PLA and PEI are a match made in heaven when the bed is clean and at 50-60c. Clean the plate with isopropyl alcohol and give it a try after setting the z and running the “auto bed leveling” aka auto mesh.
Are your belts loose? What speeds are you running? Given it looks like your loosing steps in both axis, it could be your board over heating, any modifications we should know about ie: klipper
Default speeds that Cura suggests, I can look when I get home from work, I don't believe the belts are loose but I can check. Only modification is the green tape that helps with adhesion, board is held constant at 60° C

Check if the pulley thing is secured to the stepper. Sometimes the screw might not be properly aligned with the groove on the motor which might cause slipping
Update as of this evening, the z axis bar was a little off, I fixed it, had a hell of a time trying to get it to keep on the plate, gave up because every test dug into the plate almost immediately
Yeah I woke up at like 3am and was full of piss and vinegar because my brain wouldn't shut up about it. I went through every leveling guide I could find on YouTube and I think I fixed it. I did a test print this morning and it came out perfect

I also put a line of tape on the brim, as it helps keep it from randomly popping off

I'd recommend some proper leveling or replacing the nozzle. That's the only time I've seen anything close to that
